Dr. Michelle Chaban: Clinical Psychologist


Dr. Michelle Chaban earned her Bachelors of Arts degree from the University of Kansas, and received both her Master’s and Doctorate in Clinical Psychology in Chicago, from the Illinois School of Professional Psychology. She is a licensed clinical psychologist and a member of the Illinois Psychological Association and the American Psychological Association. Dr. Chaban has been providing psychological services to individuals in the community for more than 15 years.

From the time of her intensive graduate training years to her present professional practice, Dr. Chaban has developed extensive clinical expertise working with a broad range of individuals in a variety of settings. These experiences helped shape her current interests and consequently, her diverse practice. Dr. Chaban’s expertise includes working with high functioning adults as well as people who struggle with serious mental illnesses and developmental disabilities. She has assessed and treated clients in hospitals, intermediate care facilities, correctional institutions, alternative schools for students with behavioral and emotional problems, and in her private offices.

Dr. Chaban opened her private practice in 2003 to continue her commitment to help clients manage mental illness, cope with life’s challenges, and improve the quality of their relationships and lives. She believes in empowering her clients so they can live their lives with purpose and fulfillment. Through developing collaborative relationships with clients, Dr. Chaban encourages them to take an active role in their treatment. In addition, she carefully tailors treatment for every client based on their personal goals while also being sensitive to any cultural and religious values that may affect their life and relationships.

Currently, Dr. Chaban is seeing clients at her conveniently located offices in Deerfield, Chicago, and Evanston. She also treats clients who reside in nursing homes that specialize in caring for adults with severe mental illness.

~ 60 minute session: $160
~ 45 minute session: $130
~ 30 minute session: $90
~ 60 minute family session: $160

~ Dr. Chaban accepts Aetna, Medicare, Blue Cross Blue Shield PPO, Blue Cross Blue Shield- Blue Choice PPO, Blue Cross Blue Shield- Blue Choice Preferred PPO, Humana MMAI, Humana IL ICP, Illinicare, ERS, Meridan Health, Ambetter, and Cigna.

Treatment sessions range from 45-60 minutes. Payment is due at time services are rendered. Payment schedules for unusual hardship are available upon request. Once an appointment is scheduled, clients are expected to pay for it unless they provide 24-hour advance notice. There may be exceptions to this rule if there are unique circumstances beyond one’s control which are mutually agreed upon by therapist and client.

If you have an insurance policy, it will probably cover some mental health services. We will directly bill insurance carriers such as Aetna, Ambetter, ERS, United HealthCare, Blue Cross Blue Shield, Meridan Health, Illinincare, AmbetterCigna, and Medicare since we are in network. However, we do not bill other insurance carriers. We will assist you in any way we can so you receive the benefits to which you are entitled; however, you are responsible for payment of our fees.

We also accept MasterCard, Visa, and Discover. You may fill out a form which is kept on file so we can charge your credit card for outstanding balances.
